New Patient Checks

When you register with the surgery, if you are over 40, or have complex needs, or are on regular medication, you will be offered a health check with a nurse or health care assistant, who will discuss your medical history. Please bring a sample of urine for testing at your health check.

Health Checks

We run regular health checks for coronary heart disease, diabetes, asthma, hypertension, diet and weight problems, smoking cessation, well woman and well man clinics with our nurses.

Our nurses have specialist areas, so please ask to make an appointment with the appropriate nurse.

Travel Immunisations

If you are going abroad, please fill in this travel risk assessment form at least 6 weeks before your travel date. Once complete, please send to hwh.admin@nhs.net and one of our nurses will be in contact to advise you further on what you need. Minor Operations

We periodically run a minor operation clinic following referral from your doctor. For some requirements you may be referred to The Swan Surgery, Bury St Edmunds

Minor Injuries

Patients may be seen in the surgery for a range of minor injuries, they may otherwise visit A & E departments. Either contact the surgery or visit for advice. We may still need to refer you to A & E, e.g. for suspected fractures, but many minor injuries can be dealt with by our doctors and nurses.

Smoking Cessation

The Practice offers advice to patients who wish to stop smoking, and are able to offer interventions either by our own team or by referring to services outside of the Practice.

Chlamydia Screening

As part of our services relating to Contraception and Sexual Health the Practice offers screening for chlamydia.

Cancer Care Navigator

Helen Ayers, our Cancer Care Navigator, is available at Hardwicke House once a month to help you with any concerns that a cancer diagnosis may bring. This gives you a chance to discuss your physical and emotional health and wellbeing. She has information on local activities and resources, including exercise programmes, benefits advice, support groups and complementary therapy. To make an appointment to see Helen (first Thursday of every month), contact the Cancer Care Navigator team on 01284 713595.

Childhood Immunisations

When you receive a letter from the Child Health Immunisation Service (CHIS) for your childs' periodic vaccinations, please contact your surgery to make an appointment.

Contraception and HRT

We have Doctors and Practice Nurses who are specially trained in all aspects of contraception and HRT. If you would like to have advice on any aspects of contraception or HRT please make an appointment.

Maternity Care

The local midwifery team share antenatal and postnatal care with the doctors, under the maternity services provisions.
