Hardwicke House Group Practice Patient Partnership

It is vital the Hardwicke House Group Practice has a vibrant Patient Partnership to ensure the "Patient's Voice" is heard. The Hardwicke House Group Practice Partnership, a partnership between practice staff and patients is being relaunched. Members are needed from the 5 surgeries in the practice to represent a wide range of health conditions and the interests of carers.

The West Suffolk Clinical Commissioning Group which is responsible for commissioning healthcare services in West Suffolk says it wants to put "Patients at the heart of their decision making". To ensure The Hardwicke House Group Practice "Patient's Voices" are heard it is essential there is a strong Patient Reference Group to feed into the West Suffolk Clinical Commissioning Group.

Issues the group will get involved with

  • Provide a link between the practice staff and patient views.
  • Assist with patient satisfaction surveys.
  • Provide a link between the Practice and wider community representative groups, e.g. West Suffolk Clinical Commissioning Group's Community Engagement Group, Healthwatch, Age UK etc.
  • Raise awareness of local support groups.
  • Share best practice and good ideas to enhance the well-being of patients and staff.
  • Input to Practice Information to ensure the content meets local needs and is accessible to all patients.
  • Organise patient information events, e.g. Displays in surgeries to support national awareness campaigns, and patient education events e.g. Healthy Lifestyles.
  • Provide a sounding board and input to major changes to services.

We try to encourage patients across all age groups, economic and social backgrounds to join the group. Some of the ways we have done this:

  • the above information is available in each of our surgeries in leaflet form
  • PPG notice boards in each of our surgeries
  • the group has written to local schools and colleges seeking pupils on health & social care related courses who might be interested in joining the group
  • fliers handed out at our walk-in 'flu clinics attended every year by over 2,000 patients
  • website links
  • fliers issued with repeat medication

While Group members are predominantly from older age groups, we do have representation in younger age groups who represent patients with particular health needs and it is hoped that our recruitment exercise in schools and colleges will be productive.
